Officer Change/Appointment Worksheet
(Form I-1)
- Required for new Chapter Directors, Assistant Chapter Directors, Rider Educators, Membership Enhancement Coordinators and Treasurers.

Memorandum of Understanding (MOU) - Required for new Chapter Directors, Assistant Chapter Directors, Rider Educators, Membership Enhancement Coordinators and Treasurers.

- Chapter Directors must sign as Appointing Officer for the Assistant Chapter Director they are appointing.

- MOU shall be printed two-sided.

- The new Officer (Applicant Form I-1) is the “legal” Officer recognized by GWRRA and the only person required to sign the MOU. Co-applicants are recognized as part of the team but have no legal requirements.
Oath of Office - Required for new Chapter Directors, Assistant Chapter Directors, Rider Educators, Membership Enhancement Coordinators and Treasurers.
Financial Report Cover Sheet & 
Financial Report
- Must be completed and submitted when a change of Chapter Director or Treasurer occurs. Not required for change of Assistant Chapter Director.   

- Outgoing Chapter Directors completed report from ending date of last annual report to change date. The report will be submitted as a “This is a change of Officer Financial Report”. The change report can also be completed in combination with the Annual report.   

- Completed by new Chapter Director as a “This is a change of Officer Financial Report”
Equipment List
(Form I-34)
- Must be completed and submitted when a change of Chapter Director or Treasurer occurs. Not required for change of Assistant Chapter Director.   

- Outgoing Chapter Director completes and signs Page 1 (Former Officer Signature). Provide to new Chapter Director for review and signature on Page 2 (Current Officer Signature).   

- New Chapter Director completes separate Equipment List, signs both sides and attaches to their Change of Officer Financial Report.
Bank Information and Staff Signatures - Must be completed and submitted when a change of Chapter Director or Treasurer occurs. Not required for change of Assistant Chapter Director.   

- At a minimum Staff Signatures will include Chapter Director, Treasurer and District Director.   

- Turn in with Financial Report.
IRS Form 8822 - New Chapter Director will complete Part II (Change Your Business Mailing Address), blocks 10, 11a, 11b, 12, 13, 14, Part III (Signature) daytime phone number, Part II signature, date and title.   

- When completed mail the “original” to the appropriate address on Page 2.   

- A copy of the Form 8822 must be submitted with the New Officer Paperwork.
IRS Form 8822B - It looks like IRS has a new form for Change of Address.  It’s called the 8822-B specifically for “businesses.”  When there’s a change in CD, this form will need to be filled out and mailed to the IRS.  A copy of this form needs to be included with the other paperwork that is required when there’s a change in CD. 
***** It is suggested that a new Chapter Director make copies and maintain with the Chapter files prior to submitting to the District.
